Accurate diagnosis is the foundation of effective treatment. Without precise, comprehensive, and clinically interpreted data, treatment plans are built on assumption rather than evidence. This is particularly true in hormone health, functional medicine, and longevity care, where the conditions most likely to be driving a patient’s symptoms are often invisible to standard investigations and unmeasured by routine blood panels.
At The Schoeman Clinic, diagnostics are not a box-ticking exercise. Every test we use is selected because it answers a specific clinical question, adds meaningful information to the patient’s overall picture, and directly informs the treatment plan that follows. We draw on a wide range of advanced functional, genetic, metabolic, cardiovascular, and imaging investigations, coordinated by Dr Gina Schoeman and supported by specialist laboratory and clinical partners of the highest calibre.

Standard blood tests assess a limited range of markers against broad population reference ranges. They are designed to detect overt disease, not to identify the subclinical imbalances and early functional changes that precede it. Our advanced blood work goes considerably further, assessing hormonal, metabolic, inflammatory, cardiovascular, thyroid, and nutritional markers in a clinical context that accounts for your individual symptoms, history, and health goals.
Testing is conducted through The Doctors Laboratory (TDL), a specialist pathology partner used by leading private medical clinics across the UK. Results are interpreted by your doctor within the full context of your clinical presentation, not compared against a reference range in isolation.
Hormonal testing at The Schoeman Clinic assesses the full spectrum of endocrine function relevant to your presentation. Panels are selected individually and may include sex hormones such as oestrogen, progesterone, testosterone, SHBG, and LH/FSH; thyroid hormones including TSH, free T3, free T4, and thyroid antibodies; adrenal hormones including cortisol, DHEA, and DHEA-S; and metabolic hormones including fasting insulin and IGF-1. Testing is used both to establish a baseline and to monitor treatment response over time.
Understanding your genetic architecture allows us to make genuinely personalised clinical decisions. Genetic testing may assess variants affecting detoxification pathways, hormonal metabolism, cardiovascular risk, inflammatory tendency, and nutritional requirements. This information does not determine outcomes, but it does define the biological context within which your lifestyle, environment, and treatment choices have their effect.
Epigenetic testing, most commonly through DNA methylation analysis, measures biological age at a cellular level and identifies the extent to which lifestyle and environmental factors are influencing gene expression. This is one of the most clinically valuable tools in longevity medicine, providing a direct and modifiable measure of ageing rate.
Food sensitivities differ from allergies in that their effects are typically delayed, diffuse, and difficult to attribute without targeted investigation. Symptoms associated with IgG-mediated food reactions include bloating, fatigue, brain fog, skin changes, joint discomfort, and disrupted sleep, all of which overlap substantially with hormonal and metabolic presentations and can significantly complicate clinical assessment if left unidentified.
The 184 IgG Food Sensitivity Test screens for immune reactions to 184 commonly consumed foods through a simple blood draw. Results allow us to identify and temporarily eliminate reactive foods as part of a broader gut restoration and anti-inflammatory protocol.
The gut microbiome exerts significant influence over immune regulation, hormonal metabolism, inflammatory signalling, brain function, and metabolic health. When the microbiome is disrupted, whether through antibiotic exposure, dietary patterns, chronic stress, or other factors, the downstream consequences can affect virtually every system in the body.
The GI360 test is a comprehensive at-home stool analysis using validated PCR and culture technology to assess microbial diversity and balance, detect pathogenic bacteria, viruses, and parasites, evaluate digestive function markers, and measure intestinal inflammation. It is the most clinically detailed gut health investigation we use and forms the basis of targeted microbiome restoration protocols.
Zonulin is a clinically validated marker of intestinal permeability, commonly referred to as leaky gut. Elevated zonulin levels indicate that the gut barrier has become compromised, allowing bacterial products, undigested food particles, and inflammatory triggers to enter systemic circulation. This process has been associated with autoimmune conditions, chronic inflammation, hormonal disruption, and a range of systemic symptoms that are frequently investigated without resolution.
Zonulin testing may be performed independently or alongside the GI360 analysis, and results are used to guide intestinal barrier repair protocols.
The Organic Acids Test is one of the most comprehensive functional assessments available and one of the most clinically informative tests we use in complex or long-standing cases. It analyses over 70 metabolic markers in a single urine sample, providing insight into mitochondrial function, neurotransmitter metabolism, detoxification capacity, gut microbial activity, nutritional sufficiency, and cellular energy production.
The OAT is particularly valuable in patients presenting with chronic fatigue, cognitive difficulties, mood disorders, or complex multi-system symptoms that have not been resolved through conventional investigation.
Mould-related illness is more prevalent than conventional medicine typically acknowledges. Mycotoxins, the toxic compounds produced by certain mould species, can accumulate in the body following exposure in damp or water-damaged buildings and have been associated with chronic fatigue, respiratory symptoms, brain fog, allergic responses, and immune dysregulation.
We offer validated mycotoxin testing for patients whose symptom profile and environmental history suggest possible mould exposure. Results guide a targeted detoxification and immune support protocol.
Chronic exposure to environmental chemicals, including heavy metals, plasticisers, pesticide residues, and persistent organic pollutants, contributes to hormonal disruption, neurological symptoms, metabolic dysfunction, and accelerated biological ageing. These exposures accumulate gradually and are rarely assessed in conventional clinical settings.
We use validated functional testing to assess environmental toxic burden and identify the specific compounds most relevant to your clinical presentation. A personalised detoxification protocol is then developed to support clearance through the body’s natural elimination pathways.
Intravenous nutrient therapy delivers vitamins, minerals, amino acids, and antioxidants directly into the bloodstream, bypassing the digestive tract and achieving tissue concentrations that are not attainable through oral supplementation alone. IV therapy is used at The Schoeman Clinic as a supportive intervention in the context of a broader treatment plan, not as a standalone treatment.
Clinical indications may include nutrient depletion identified through testing, chronic fatigue and mitochondrial support, immune resilience, post-viral recovery, detoxification support, and preparation for or recovery from significant physiological stress. At The Schoeman Clinic, lifestyle medicine is not a supplementary suggestion offered at the end of a consultation. It is a core component of every treatment plan, delivered with the same clinical rigour and personalisation as pharmaceutical and nutritional interventions.
Lifestyle support encompasses personalised nutritional guidance informed by your test results and genetic profile, sleep architecture assessment and optimisation, circadian rhythm medicine, stress physiology, and practical guidance on movement and recovery. Where specialist nutritional support is required beyond what we provide in-house, we refer to our nutritional therapy partners, including IFM Certified Nutritionist Jackie McCusker and BANT Registered Nutritionist Mays Al-Ali.
The relationship between psychological stress and physical health is not metaphorical. Where patients would benefit from dedicated psychological support, we work within a network of trusted referral partners.
Sleep is one of the most powerful biological levers available in longevity and functional medicine. Chronic sleep disruption impairs hormonal balance, metabolic regulation, immune function, cognitive performance, and cardiovascular health. Many patients live with undiagnosed sleep disorders, including obstructive sleep apnoea, that significantly compound their presenting symptoms without ever being formally identified.
We arrange sleep studies for patients where clinically indicated, providing detailed analysis of sleep architecture, respiratory events, oxygen saturation, and other parameters. Results are reviewed by your doctor and integrated into the broader treatment plan.
Cardiology diagnostics.
For patients requiring specialist cardiovascular investigation, The Schoeman Clinic works in partnership with One Heart Clinic, a specialist cardiology service. Cardiovascular risk is a central concern in longevity medicine, and early, accurate detection of arterial disease, haemodynamic abnormalities, and functional cardiac capacity allows us to intervene long before clinical events occur.
CT coronary angiography (CTCA)
A non-invasive scan that provides detailed images of the coronary arteries, identifying narrowing, calcification, or plaque burden that significantly increases the risk of heart attack or stroke. It is one of the most sensitive tools available for early cardiovascular risk stratification and is particularly valuable in patients with a relevant family history, metabolic risk factors, or atypical symptoms.
Ambulatory blood pressure monitoring (ABPM)
A single blood pressure reading taken in a clinical setting is subject to significant variability and does not reliably reflect a patient’s true haemodynamic profile. ABPM records blood pressure at regular intervals over a 24-hour period while the patient goes about their normal daily activities, providing a far more accurate and clinically meaningful picture, particularly in assessing cardiovascular risk and in monitoring the effectiveness of antihypertensive strategies.
Cardiopulmonary exercise testing (CPET)
CPET measures cardiovascular, pulmonary, and metabolic function simultaneously under graded exercise conditions, providing an exceptionally detailed picture of functional capacity and physiological reserve. It is used both as a risk stratification tool and as a baseline for personalised exercise prescription within a longevity programme.
Advanced heart imaging
Advanced cardiac imaging beyond standard echocardiography provides detailed assessment of cardiac structure, function, and vascular health, supporting both early detection and ongoing monitoring of cardiovascular risk.

Full-body MRI
We offer access to comprehensive full-body MRI scanning, combining high-resolution magnetic resonance imaging with optional low-dose CT add-ons. This non-invasive investigation provides a detailed internal picture that goes well beyond the scope of standard diagnostic imaging, supporting early detection of structural abnormalities, organ changes, and incidental findings across multiple body systems.
Pelvic ultrasound
A safe, non-invasive imaging modality that provides detailed visualisation of the uterus, ovaries, and surrounding pelvic structures. In the context of hormone replacement therapy and women’s hormonal health, it plays an important clinical role in monitoring endometrial thickness, detecting fibroids or ovarian cysts, investigating irregular bleeding, and providing reassurance throughout the course of treatment. Pelvic ultrasound is available as a standalone investigation and as part of a broader women’s health assessment.

The Schoeman Clinic offers Trucheck Intelli, a clinically validated blood test capable of detecting circulating tumour cells associated with more than 70 types of solid organ cancer, including many that fall outside the scope of standard NHS screening programmes. Trucheck is recommended as part of a comprehensive preventative health strategy, particularly for adults over the age of 40 or those with relevant family or lifestyle risk factors.
Trucheck does not replace NHS screening but complements it, providing broader coverage and earlier detection capability. The test involves a simple blood draw and results are available within two weeks, reviewed with your doctor in a dedicated follow-up consultation.
